Family Medicine

El Rio’s Family Medicine Primary Care Providers (PCPs) offer healthcare to men, women and children of all ages.  They provide a wide range of services, which include: chronic disease management, patient education and preventive care as well as physicals, well-child exams, and annual gynecological screenings. These physicians also provide continuity of care for patients struggling with chronic health issues.

Internal Medicine

El Rio’s Internal Medicine PCPs focus on adult health care for men and women who are 18 years of age and older.  They offer a wide range of services, including: preventive care, behavioral health care and substance abuse treatment.  In addition, they provide inpatient care to El Rio patients hospitalized at Carondelet St. Mary’s Hospital.

El Rio’s Pediatricians care for children of all ages (birth to 18 years). Care includes sick and well-child visits, physical exams, and sports physicals, as well as disease management and education for children suffering from chronic illnesses such as asthma and diabetes.

Clinical Pharmacy

El Rio’s Pharmacy-Based Diabetes Clinics provide a unique, award-winning approach to diabetes management using clinical pharmacists to provide comprehensive medical treatment based on effective drug regimens. This program has three distinctive features:

  1.  increased direct services to patients involving intensive and ongoing consultation with a clinical pharmacist (in Spanish, when needed)
  2.  integrated treatment of three related diseases (diabetes, high blood pressure, and high cholesterol)
  3.  in-depth information and education which empowers patients to proactively manage their health

Behavioral Health

We have behavioral health specialists who not only focus on your mental health problems but also the impact these issues can have on your physical health. We care about making you better – both in body and mind.

Patient Care Coordination

In order to maximize patient care coordination, El Rio has adopted the Patient Centered Medical Home model throughout our clinical sites.  A core function of the Patient Centered Medical Home (PCMH) is to organize patient care activities (between two or more participants involved in a patient’s care) in order to facilitate the appropriate delivery of healthcare services and eliminate confusion from the patient’s perspective. –

Nationally Recognized Organization

El Rio Community Health Center has demonstrated its commitment to your health through certification by two of the most prominent healthcare quality organizations in the United States.

The National Committee for Quality Assurance (NCQA) has granted El Rio a Level 3 Patient-Centered Medical Home Certification.The program organizes care around you, the patient, working in medical teams and coordinating and tracking your care over time. This healthcare process facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family.
