The
Eligibility Outreach Workers (E.O.W.) will assist our
community members to locate available resources for
health care.
The eligibility workers have extensive knowledge of
state health care programs and discount programs.
The E.O.W. will screen patients for potential
eligibility with different programs.
If the patient is eligible for state programs the
E.O.W. will submit the application on line through Health-e-Arizona,
Arizona’s electronic health care
application developed by El Rio and other collaborators.
Patients can learn if they and/or other family
members are eligible for state and community programs
such as
AHCCCS,
KidsCare,
PCAP,
or a sliding-fee scale.
If the client encounters barriers with the
application or documentation needed the E. O. W. will
assist with those barriers.
The E.O.W. will also educate and inform the
patient on how to access services and how their plan or
discount programs works.
Once the health care program has been identified
for the patient and/or their family members; they may
register with El Rio Health Center for both medical and
dental services through their health plan or at
discounted fees.
Our OB Monitors have extensive knowledge of state
health care programs and resources available to assist
OB patients with health care coverage.
They will screen OB patients for prenatal care coverage through state
programs using Health-e-Arizona.
If a patient is determined ineligible for state
programs; El Rio offers prenatal care through discount
pricing. The
fees charged for prenatal care are based on household
income.
Patients requesting prenatal care through El Rio have
the choice of services through our Midwives department,
Birth and Women’s
Health Center or our OB/GYN facility.
NOTE:
If you would like to apply for our
sliding fee or other health care program please bring
with you: proof of income for current and previous
month, proof of address (gas, electric or water bill),
and birth certificates/INS status/Social Security
numbers for all family members.
