The Patient Portal is a secure, confidential, and easy to use website that gives you 24 hour access to your personal health information and medical records. It uses the latest encryption technology to deliver secure communication between patients and providers. The User Guide is located HERE.
By signing up and enrolling in the Patient Portal, you will be able to:
You will receive enrollment instructions during your next visit to El Rio Community Health Center. If you give the front desk an email address when you are checking in, you will receive an email with a link that will allow to you complete your enrollment.
To access the Patient Portal, go to www.nextmd.com. Enter your Username and Password. If you do not remember your login information, click on “Need help with your username and password.”
Click on My Chart tab and Request Health Record. Your lab results should load within approximately 24 hours as long as your doctor has signed off on the lab results. You can view your lab results from the Home Page or click on My Chart and select View My Chart.
Your Patient Portal health information comes directly from your electronic medical record at your doctor’s office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.
You should expect to receive an answer within 1 to 2 business days. Please note that the Patient Portal should not be used for urgent situations. Please dial 911 if it is an emergency.
When a new message arrives in your Patient Portal inbox, an email alert will be sent to the personal email address you provided when you set up your account.
Parents and legal guardians of children under 12 years of age can request proxy access to their child’s account by informing the clinic. To comply with state and federal privacy laws, access will be discontinued when a child turns 12 years of age. Parents of children with documented incapacitation can request an exception to this policy.
Because we encourage our teenage patients to take an active interest in their health, patients 12 years of age and older can create their own Patient Portal account.
When a child becomes an adolescent, some information in the medical record can become protected or confidential by law. To ensure confidentiality of these conversations and comply with state and federal privacy laws, El Rio does not provide proxy access for Portal accounts of minors aged 12 to 17.
Your spouse must fill out a Release of Information form in Medical Records before providing proxy access to another patient’s portal account.
